Monday Clinic:  Free Childrens Immunizations

Ages:  infant to 6 yrs.

Must bring shot record

8:00 am to 11:00 am and 1:00 pm to 3:00 pm

 FREE Immunizations for Grades 6-12    (ages 11-18)           

In 2009, the Indiana State Legistation revised the Indiana Code to require all 6-12 students to receive additional immunizations against the following diseases.

1)  Meningitis        2)  Varicella (Chicken Pox)   3)  Tdap (Tetanus, Diptheria and Pertussis)

Tuesday thru Fridays  8-11 am and 1-3 pm

 ***Parents will need to bring their child's shot record and child must be accompanied by parent or legal guardian.*******
